We advise you to read through the 2021-FAQ before you attend orientation. This information will be emailed to you about a week ahead of the orientation date.  Please read over and bring any questions you have to orientation.

#1-Fill these forms our during orientation.

• 2021 Commitment Contract and Acceptable Use Policy-this forms needs to be completed BEFORE students are allowed on the computers in the classroom.
• The 2021-16-year-old-Form if applicable, this form needs to be printed and completed before the student may enroll in classes. The student MAY attend Orientation before getting this form completed. This form can be mailed in to the office address ( ATTN:  Joan Wright, 128 Garfield Avenue, West Plains, MO  65775) or scanned and emailed in to the teacher.
• The 2021-Home-School-or-Drop-Out-Compliance-Statement if applicable, this form needs to be printed and completed before the student may enroll in classes. The student MAY attend orientation before getting this form completed. This form can be mailed in to the office address ( ATTN:  Joan Wright, AEL Director, 128 Garfield Avenue, West Plains, MO  65775) or scanned and emailed to the teacher.

#2-Fill these forms out before attending class. 

• Enrollment form-this form will be filled out during the individual conference with your teacher.  Test scores must be added to this form, so please fill out with the assistance from your teacher.

#3-Fill these forms out during class after enrollment is complete.
